First, Second, and Third Class medical certificates with Dr. Kirk A. Paulk, MD, Aviation Medical Examiner. We do not provide HIMS or BasicMed services.
Before booking: complete your application at medxpress.faa.gov and enter your confirmation number under this form. We cannot begin the exam without it.
Fee
$250.00 — any class
$300.00 — if an EKG is required
Card, cash, or check, due at the time of service. No additional fees. Not billed to insurance.
The fee covers the examination. Not every exam results in same-day issuance — some cases must be sent to the FAA for review. The fee is the same either way.
Monday–Thursday, 9:30 am–4:00 pm. Closed Fridays. Plan on 30–45 minutes.
